I hereby give my permission for my child or children to participate in Code Rev Kids camps and classes. I hereby waive, release and absolve Code Rev Kids, Inc., its officers, agents, employees and all other participants in this program, from any and all claims, rights or causes of action, whether for property damage or personal injury, which may arise out of my child or ward’s participation in all recreational and learning activities in Code Rev Kids classes and camps.

Further, I hereby agree to defend, indemnify and hold harmless Code Rev Kids, Inc., its officers, agents, employees and all other participants in such activity from any claim arising out of injury to my child or ward, arising from his or her participation in any way in the activity(ies) described on this form and program website.

All pictures, films, tapes, or other likenesses of my child taken during Code Rev Kids, Inc. hours may be used by Code Rev Kids, Inc. for promotional materials so long as my child’s full name is not identified in writing by Code Rev Kids, Inc. If I choose not to allow Code Rev Kids, Inc. to use any images taken for the above purposes, I will notify Code Rev Kids, Inc. in writing by email to register@coderevkids.com
